We are actively seeking a Manager of Case Management RN to join our dynamic team. This role is critical to ensuring seamless coordination of medical services and effective discharge planning for our patients.

As the Manager of Case Management RN, you will play a key role in reviewing patient medical records for compliance, collaborating with patients, families, medical staff, and community agencies, and ensuring continuity of care.
