Home Health Physical Therapist - Bergen County, NJ
Job Type : Full-Time / Part-Time / PRN
Shift : Standard Schedule
Job Summary :
Performs Physical Therapy assessments of patients in their homes. Prepares clinical documentation including assessments, visit and progress notes, and coordinates services as directed by the Case Manager and / or Patient Care Coordinator.
Submits all documentation within timelines determined by the agency. Evaluate outcomes of care. Provides patient and family education.
Qualifications / Requirements :
Graduation and Diploma from an Accredited Physical Therapy Program.
Current State of New Jersey Physical Therapy Licensure
Valid driver's license and automobile liability insurance
Current CPR certification preferred
Minimum 1 year of experienced is required (Preferably in Home care)
Additional Requirements :
Should be available between 11 am to 7 PM Monday thru Friday
Should be willing to travel through Bergen County, NJ to see patients
Should have their own car
