Skilled Nursing Facility (SNF) experience preferredLicense : State License requiredCertifications : COVID-19 test 72 hours prior to start date with results issued 48 hours priorMust-Have : - State License Verification- Resume- References (3)- Background Checks (County and Federal Criminal, Multi-State Criminal, Nationwide Sex Offender search)- Health and Human Services (HHS) OIG search- General Services Administration (GSA) search- Social Security Number Trace- 2 Step PPD or Quantiferon or proof of positive PPD and chest X-ray- System for Award Management (SAM) searchJob Description : - Ensure prescreen is completed and attached when submitting a candidate- Include resume and license verification in the submission- Candidates without the required documents will be declined- Perform physical therapy services and patient assessments - Develop and implement treatment plans for individuals requiring rehabilitation therapy- Collaborate with a multidisciplinary team to optimize patient outcomes- Document patient progress accurately and adjust treatments as necessary- Maintain compliance with all safety and health protocolsPlease ensure the credentialing checklist is reviewed prior to submission.
