- Proof of vaccination must include the vaccine type, lot number, expiration date, date and site administered.
- The full name of the employee must also be visible in the submitted record.
- If the vaccines were administered when the employee was a child and is applying with a different name, (married, etc.) then this must be indicated in the documentation.
- As an alternative titers from a diagnostic lab indicating appropriate level of immunity can be submitted.
