Join the dedicated Geriatrics team of providers at Corewell Health West providing care to complex patients residing in long term care facilities in Grand Rapids, MI.

If you have a passion for helping patients and families manage complex, multi-dimensional challenges in a variety of settings, this is the opportunity for you!Provider will spend dedicated days at specific Grand Rapids area Corewell Health Rehabilitation and Nursing Facilities .

The APP will provide care for patients three days (12 hrs / day) per week. Care inclusive of health assessments and treatment recommendations to residents.

Excellent physician support and great relationships developed with consistent scheduling at facilities.Enjoy being part of a collaborative & multidisciplinary care teams.
