Experienced Chronic Care Manager Supervisor - Vitability Health New Jersey

Job Overview :

We are seeking an experienced and dedicated professional to join our healthcare team as a Chronic Care Manager Supervisor.

The ideal candidate will have a minimum of 3 years of hands-on experience in chronic care management, coupled with at least 1 year of supervisory experience.

In this role, you will be responsible for overseeing and guiding a team of chronic care managers to ensure the delivery of high-quality, patient-centered care for individuals with chronic health conditions.

Responsibilities :

Supervision and Leadership :

Provide leadership and guidance to a team of chronic care managers.

Conduct regular team meetings to facilitate open communication, share insights, and address challenges.

Foster a culture of collaboration, continuous learning, and excellence within the team.

Chronic Care Management :

Utilize your expertise in chronic care management to support the development and implementation of care plans.

Oversee the development and implementation of individualized care plans for patients with chronic conditions.

Ensure that care plans are tailored to meet the unique needs of individuals with chronic health conditions.

Proactively identify and address barriers to effective care coordination.

Stay abreast of the latest developments in chronic care management and incorporate best practices into team processes.

Quality Assurance :

Monitor and evaluate the quality of care provided by the chronic care management team.

Implement and oversee quality improvement initiatives to enhance patient outcomes.

Ensure compliance with relevant healthcare regulations and standards.

Team Training and Development :

Conduct ongoing training sessions to keep the team updated on the latest advancements in chronic care management.

Identify and address training needs to enhance the skills and knowledge of team members.

Collaboration :

Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive and coordinated care.

Establish and maintain effective communication channels with internal and external stakeholders.

Data Management :

Oversee the collection and analysis of data related to chronic care outcomes.

Use data-driven insights to identify areas for improvement and implement changes as needed.

Qualifications :

Bachelor's degree in nursing, healthcare management, or a related field; Master's degree preferred.

Minimum of 3 years of direct experience in chronic care management (CCM), with a proven track record of positive patient outcomes.

At least 1 year of supervisory or managerial experience in a healthcare setting (CCM supervisor preferred).

Strong understanding of chronic health conditions and evidence-based interventions.

Excellent communication, leadership, and interpersonal skills.

  • In-depth knowledge of chronic care management protocols, best practices, and emerging trends.
  • Ability to lead a team effectively, foster collaboration, and create a positive work environment.

Familiarity with healthcare regulations and compliance requirements.
