We are seeking a Director of Social Services available to work full time at our Skilled Nursing Facility.

Provides supervision to the social work staff and evaluates their performance in accordance with facility policy
Responsible for assessments, care planning and helping residents adjust to their new surroundings
Arranges admission interview with residents, and / or family members, to gather psychosocial information necessary to completion of the initial assessment
Develop and execute individual treatment plans
Communicate updates to all relevant parties
Facilitate referrals to other healthcare professionals and programs
Formulates the department budget and ensures appropriate maintenance and monitoring of assigned budget
Provides supportive social work services to help residents adjust to their illnesses, disabilities and stay in the facility
Provides consultation to facility staff regarding the social and emotional aspect of resident health care and treatment
Modifies social service goals or treatment consistent with change in resident care needs
Prepares a variety of records and reports related to the work
Audits charts for social work documentation, MDS completion and appropriate care planning to assure quality and compliance with mandated documentation requirements
FULL PERFORMANCE, KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS :
- Good knowledge of the practices and procedures of social work and of the social and psychological factors related to disease and disability;
- working knowledge of State and Federal rules, regulations and requirements related to health care facilities; ability to counsel health care facility patient and their families;
- ability to function as a member of an inter-disciplinary health care team; ability to interpret Federal, State and local laws and regulations regarding the delivery of social work services in a health care facility;
- ability to plan and supervise others; ability to prepare records and reports related to social services activities; ability to utilize community resources effectively to meet facility patient and family social service needs;
- ability to confer with and provide social services consultation to other health care professionals; sympathetic attitude toward the sick and disabled;
tact; physical condition commensurate with demands of the position.
QUALIFICATIONS : REQUIREMENTS :
Previous experience in social work, counseling, or other related fields
Compassionate and caring demeanor
Ability to build rapport with residents
Strong leadership qualities
Excellent written and verbal communication skills
REQUIREMENTS :
MSW from an accredited school of social work
Minimum 2 years of management experience in a LTC or SNF setting
