Nurse Practitioner - School Based Asthma Therapy

Practices in accordance with the philosophy, policies, procedures and standards of NCH. Functions within the APN practice guidelines agreed upon by the medical and patient care services departments;

performs all patient management activities in conjunction with the collaborating physician in accordance with their Standard Care Arrangement (SCA).

The Nurse Practitioner for the School-Based Asthma Therapy (SBAT) Program assesses, plans, implements, and evaluates the School-Based Asthma Therapy Program.

The Nurse Practitioner will obtain appropriate consents to participate, assess and discuss asthma plan of care with the family, communicate with school nurses and Primary Care Providers, and visit area schools.

In collaboration with the Physician Lead and the Primary Care Provider, the Nurse Practitioner contributes to an appropriate plan of care for asthma including medication management, in accordance with APN practice guidelines and in conjunction with the collaborating physician for patients enrolled in the SBAT program.

Demonstrates competencies defined in APN responsibilities in providing age-appropriate, disease-specific care to asthma patients.

Provides family-centered care to meet other family needs such as psycho-social support, education, and discharge planning in conjunction with the PCP, School-Based Asthma Therapy team, and school personnel.

Initiates and participates in education, research with evidence based practice, professional, and organizational activities that contribute to improving patient care, the scientific knowledge base of nursing, program operation, or their own professional development.

Also mentors others in the area of clinical competence and professional development.

Responsibilities

1.Program Development

a.With guidance from the Lead APN :

i.Manage implementation of program at new schools

ii.Communication with school nurses and patient identification

iii.Obtain parent consent to participate in the SBAT Program

iv.Obtain insurance prior authorization for needed medications

v.Transport medications to the school

vi.Follow-up with school nurses, families and primary care providers regarding patient’s response to program; keep records regarding patient progress;

initiation of therapy change if needed

vii.Recruit new schools for participation in the SBAT Program with support from the APN and physician lead

2.Education

a.Acts as clinical resource for school nurses and families in new and established schools participating in the program

b.Assists in assessing the continuing education needs of the health care team including the School-Based Asthma Therapy RN, school nurses, school Medication Monitors, and other staff regarding asthma care techniques.

c.Maintains clinical competency, and stays informed of current trends in their specialty and in nursing practice.

d.Participates in planning and presenting educational programs for the health care team, Primary Care Providers, school nursing staff, parents and families, and other community agencies.

e.Plans and participates in the orientation of the School-Based Asthma Therapy RN, school nurses, and staff new to the School-Based Asthma Therapy program

f.Remains informed of current research specific to the care of asthma patients, making this data available to staff and applying it as appropriate as the School-Based Asthma Therapy Program grows and evolves.

Also continues to look for evidenced based practice.

3.Patient Management :

a.Functions within the APN practice guidelines agreed upon by the medical and patient care services divisions and in conjunction with the collaborating physician outlines in their Standard Care Arrangement.

i.Performs diagnostic and therapeutic procedures, according to established guidelines, as necessary for care of the patient.

Such procedures are based in their specialty area but may include : administration of emergency medications; determination of need for respiratory assistance, and selection of appropriate technique and initiation of appropriate therapy.

ii.Based on physical examination and other data in collaboration with the physician, formulates a plan of care with expected outcomes;

implements, evaluates, and revises as needed.

iii.Documents and effectively communicates the plan of care and patient progress in the medical record. Communicates the plan appropriately to the primary care physician or referring physician if applicable.

iv.Initiates consults to other services at the direction of their collaborating physician.

v.Functions in collaborative role with the professional nursing staff and other care providers sharing information and responding to concerns of the other care providers.

vi.In conjunction with the School-Based Asthma Therapy team.

4.Meets patient / family needs identified in the care plan and contributes to the care plan as appropriate.

5.Meets specific emotional / support needs of families and facilitates resolution of the parents’ psychosocial concerns, initiating interdisciplinary involvement as indicated.

6. Participates in family teaching and as necessary ordering durable medical equipment.

7..Communicates with the health care team, including the school nurse, data relevant to patient and or family needs.

8.Perform asthma follow-up visits at school according to SBAT algorithm for care.

9.Medication Management :

a.Prescribe within the Ohio Board of Nursing formulary and the CHI formulary.

a.Communicates with PCP to manage child’s medications

b.Collaborates with PCP to prescribe medication

c.When PCP agrees to participate in the Program, communicates with PCP to manage / adjust child’s medications

d.When appropriate, reconciles home and school medications

5.Professional Development and Continual Learning

a.Establishes professional objectives, evaluates self, and shares progress periodically with the Physician Lead.

b.Maintains involvement with professional nursing organizations or specialty organizations to promote collegiality and networking.

c.Assists in educating others regarding the role of the APN.

10.Other

a.Contributes to the goals of the hospital, Community Wellness, and the School-Based Asthma Therapy Program.

b.Assumes other duties and responsibilities as assigned.

Qualifications

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

1. Master of Science Degree in Nursing or must fall under the grandfather clause for APNs Registered Nurse with current licensure to practice nursing in the State of Ohio.

2. Certificate of Authority to practice as an Advanced Practice Nurse in the State of Ohio.

3. Certificate to prescribe. Valid Ohio Driver License. Minimum of three years nursing experience.

4. Demonstrates nursing competence with specific clinical skills and knowledge in asthma disease management.

5. Demonstrates leadership, interpersonal skills, teaching ability, advanced level of critical thinking and decision making.

6. Excellent verbal and written communication skills. 7. Reliable transportation some travel is required for this position.

MINIMUM PHYSICAL REQUIREMENTS

1. Must be able to see, hear, stand, walk, speak, read and perform manual tasks with or without accommodation, and care for oneself with little or no difficulty.

2. Must be able to independently lift up to 25lbs.

3 Must be able to move or reposition patients of any weight or size with the assistance of another person(s) and / or equipment.
