Job Summary
Under minimal supervision, provides individualized telephonic and / or face to face transitional case management services for individuals who require intensive support and monitoring, including medical, behavioral, and / or substance abuse services on a time-limited basis during transition from critical events such as incarceration, homelessness, and / or drug use.
Assists and educates patients to seek appropriate care and services at Denver Health for outpatient follow-up and enrolling in care.
Works closely and collaboratively with community-based organizations, clinical providers, medical systems and interdisciplinary care teams to outreach, engage and advocate for the provision of timely, cost effective use of medical care and behavioral health services to facilitate high quality outcomes.
Provides support to the Correctional Transitions Clinic (CTC) and CTC team members in assisting with patient recruitment and referral, program development, and workflow for new projects as directed.
The case manager will be part of the Infectious Diseases (ID) Clinic which is one of four clinics within the Public Health Institute at Denver Health.
The ID Clinic team is strongly committed and passionate about providing HIV care and prevention services to an underserved community including LGBTQIA+ persons.
This position will be dedicated to our low-barrier drop-in clinic for people with HIV and substance use who have complex needs and history of medication adherence difficulties.
The optimal candidate will work well within a team, willing to provide intensive case management, and have a passion for improving the lives of all people with HIV."
This position is FTE, 32 hours per week.
Essential Functions :
- Provide expanded transitional case management services for transitions from incarceration, rehabilitation facilities, and / or homelessness to Denver Health's clinical resources (20%)
- Establish / strengthen the relationship between the individual and the support services needed after reentry, housing security, and / or clean, sober living (20%)
- Direct the selection of goals, interventions, and services based on patients' needs (20%)
- Provide wrap-around services to ensure a successful transition back into the community (20%)
- Create processes for individuals becoming Denver Health patients so they are connected with Enrollment Services, the Appointment Center, and other departments, as needed. (20%)
Education :
Bachelor's Degree in Psychology, Sociology, Criminal Justice field, preferred, OR a closely related behavioral health field Required
Work Experience :
years Three- five years of experience working with diverse populations in a correctional facility, homeless shelter, rehabilitation facility or experience working in a healthcare facility that serves challenging (homeless, violent behavior, addictions, corrections, substance abuse, etc.) patients. Required
Knowledge, Skills and Abilities :
- Intermediate knowledge of the correctional and / or justice department(s)
- Intermediate knowledge of the community resources needed for reentry at Denver Health's integrated care system
- Ability to create and strengthen partnerships in the community and seek out resources for inmates upon reentry
- Strong oral and written communication skills
- Proven ability to work effectively, both individually and as a team, with populations that have been incarcerated, mental health and substance abuse issues, violent behavior, and / or addiction tendencies
- Must be empathetic to needs of inmates, homeless individuals, and those in rehabilitation facilities
- Knowledge of community resources available to challenging populations
- Subject matter expert for various community advisory councils that serve to better connect corrections and Denver Health
- Bilingual in English / Spanish preferred
- Knowledge of Epic and other electronic health records systems
- Knowledge of Microsoft Office suite products
