Schedule :
- Full-time (Monday-Friday, 8am-5pm)
- No weekends, holidays, or on-call

Responsibilities :
- Care for high-risk patients in senior living facilities
- Conduct assessments
- Manage medical and behavioral conditions
- Collaborate with care team
- Educate and counsel patients
- Discuss advanced illness and care planning
- Maintain accurate medical records
Qualifications :
- Active state license (NP / PA)
- Master's in Nursing (NP) or Physician Assistant degree (PA)
- National board certification (specific certifications listed)
- Experience with complex patient needs
- Strong communication and collaboration skills
- Proficient in electronic medical records
- COVID-19 vaccination required
