Position Overview
As a Licensed Practical Nurse (LPN), youll be responsible for the care and well-being of our residents during your assigned shift.
Youll ensure compliance with Federal and State regulations, as well as our communitys policies and procedures, all while delivering compassionate and high-quality care.
Key Responsibilities
Resident Care :
- Administer medications as prescribed by physicians.
- Monitor and ensure quality care throughout your shift, including complete and accurate documentation of resident care.
- Safeguard resident rights, ensuring their dignity and privacy.
Team Leadership :
- Orient, train, and mentor staff, identifying learning opportunities and providing on-the-job teaching.
- Monitor staff performance, offering praise and guidance. Address disciplinary actions fairly and consistently.
- Manage shift schedules, including handling call-ins, breaks, and ensuring proper staff coverage.
Safety and Compliance :
- Ensure compliance with OSHA mandates on workplace safety, including infection control procedures.
- Oversee immediate first aid and proper documentation in cases of workplace exposure.
- Implement restorative care measures such as range of motion exercises and proper body mechanics.
Emergency Preparedness :
- Participate in the Community Fire and Disaster Plan, including leading evacuation drills and coordinating resident transportation.
- Address employee problems and complaints swiftly and effectively.
- Maintain strict confidentiality and adhere to all privacy laws regarding resident and employee records.
Administrative Duties
- Document all care and services provided, including subjective findings, interventions, and resident responses.
- Prepare and update comprehensive resident Personal Service Plans as needed.
- Communicate effectively during shift changes, noting significant changes in residents' conditions or any incidents that require attention.
- Report essential information to the Wellness Director, including regulatory compliance issues, staffing needs, and opportunities for process improvements.
Clinical Responsibilities
- Provide direct patient care, administer medications, and coordinate treatments according to physician orders.
- Conduct physical assessments for new admissions and residents experiencing changes in condition.
- Develop and update service plans in collaboration with residents and their families.
- Ensure medications are administered safely and timely, and maintain proper storage and documentation of all medications.
- Attend in-service education programs to stay updated on best practices.
Supervisory Responsibilities
- Directly supervise Personal Care Assistants (PCAs), ensuring adherence to company policies and state regulations.
- Assist in training, planning, assigning, and directing work while supporting performance appraisals, rewards, and disciplinary actions.
Qualifications
- Education : Graduate of an accredited nursing program.
- Experience : At least one year of direct care experience in long-term care, acute care, or geriatric nursing is preferred.
Previous supervisory experience is a plus.
- Skills :
- Knowledge of federal and state regulations.
- Strong decision-making skills and ability to manage multiple tasks under pressure.
- Excellent communication skills, both written and verbal.
- Basic math skills for handling medication dosages and documentation.
Certificates, Licenses, Registrations :
- Current active Indiana Nursing License.
- CPR Certification.
