Job Title : Registered Nurse (RN) Case Manager - Emergency Department
Work Schedule : Three 12-hour shifts (7am to 7pm)

What to Expect in This Role :
- Jump into Action : Assess patients’ needs from the moment they arrive, developing and coordinating personalized care plans throughout their hospital stay.
- Dynamic Collaboration : Work closely with a stellar healthcare team to address every aspect of patient care in the bustling emergency department.
- Patient Advocacy : Be the champion for patients, ensuring they receive the best possible care and access to resources.
- Educate and Empower : Provide essential information and support to patients and their families, guiding them through their healthcare journey.
- Resource Optimization : Monitor and manage healthcare resources to enhance patient outcomes and reduce costs in the high-energy emergency setting.
- Smooth Transitions : Facilitate timely and safe discharge plans, helping patients transition seamlessly to the next level of care.

EDUCATION / EXPERIENCE REQUIRED :
Have graduated from an accredited BSN program.
Possess RN licensure in the state of Michigan.
Possess a minimum of three years of acute care / inpatient nursing experience.
Inpatient Case Management and / or discharge planning and care coordination experience strongly preferred.
Can communicate effectively with patients, families, and hospital personnel, have strong critical thinking and creative problem solving skills and excel at planning.
Are self-motivated and enjoy working in a fast-paced environment.
