Licensed Practical Nurse (LPN)

Position Details :

• Adult Patient
• Acuity Level : G-tube
• Location : Chandler
• Schedule : Wednesday and Thursday, 10a-6p

About case :

Patient is an adult male who is needing help 2 days a week. The shifts are 8 hours long, twice a week. The patient has a g-tube and is low acuity but does require some lifting with the help of a Hoyer lift.

Responsibilities :

• Utilizes the nursing process to assess, plan, implement and evaluate patient care
• Performs focused interview to identify specific patient needs
• Assess signs and symptoms indicating physiologic and psycho-social changes in the patient’s condition
• Collects, analyzes, and interprets data and information from health care members and documents actual and / or potential nursing diagnoses
• Other Licensed Practical Nurse (LPN) duties as assigned
