JOB DESCRIPTION : The Case Manager / Housing Specialist has on call responsibilities and is a remote case management position working primarily in the field, assisting people we support with assessment, housing searches and placement, community linkage, life skills training and support.
This individual will leverage motivational interviewing and person-centered strategies to help individuals develop and work to achieve personal goals and hope for the future.
The primary responsibility is to support homeless Youth to move out of homelessness to safe, stable housing and maintain that housing to avoid recurring episodes of homelessness.
Essential Functions T he responsibilities of the position shall include, but are not limited to :
- Work collaboratively with community partners
- Work effectively with homeless individuals and families to develop and act on an individualized services plan.
- Housing searches and placement in partnership with people we support and community partners
- Linkage to community supports, services and entitlements in line with the shared goals and vision of the individual served including supports to : Attain and maintain housing Access benefits Health Services (Medical, dental, vision, Mental, addiction, victim services, etc.
Employment and / or education Transportation Skills development
Utilize clinical support to assist individuals in overcoming barriers, working through setbacks, and developing and maintaining hope and a personal vision for a better tomorrow.
Required Knowledge, Skills, and Abilities :
- Work collaborative with community partners to identify and support those we will support through this project
- liaison with community providers and individuals serviced to aid in coordinating services to the participants as requested.
- Produce oral and written reports to address internal and external information needs as assigned.
- Maintain up to date case records
- Work collaboratively, respectfully, and effectively with individuals served, homeless services team and community services providers
- Attend and actively participate in CoC and human services meetings, events and activities
- Understanding of and ability to provide : Motivational Interviewing, Person-Centered clinical skills, Housing first approach
- Attend and gain knowledge and experience in line with HUD and CoC defined skills, training, and best practices.
- Ability to gain and demonstrate a thorough understanding of St. John’s
Community Services policies and procedures
Ability to act as a representative of St. John’s Community Services to the
public.
- Ability to be on call 24 / 7 for program support
- Ability to establish and maintain effective working relationships with associates, subordinates, representatives from other agencies, and the public.
- Ability to use sound judgment when making decisions. 14.Perform other duties as assigned.
Physical and Environmental Conditions : While this position will have a home office, it will be primarily provided in the field, meeting with individuals in their home, supporting community linkage and regular use of a personal vehicle.
The work requires no unusual demand for physical effort but is an active position.
Work environment involves risks or discomforts that require special safety precautions safely operating a motor vehicle, exposure to aggressive human behavior.
Qualifications
A bachelor’s degree (QHP preferred) in Psychology, Counseling, Human Services or a related field and a minimum of three (5 preferred) years’ experience providing services to Transitioning Age Youth (18 25) experiencing homelessness, and co-occurring illness including mental health, addiction, etc.
Excellent written and oral communication skills are required. An understanding of housing first strategies, person centered supports and motivational interviewing are preferred.
