Job Summary
Field-based, making Primary Care House Calls, Conduct comprehensive assessments, develop care plans, and provide continuous care for older adults in their homes, Collaborate with a multidisciplinary team to deliver integrated care.
Utilize your expertise in geriatrics and primary care to make a meaningful difference in the lives of your patients, Fluent Spanish is ideal.
Job Responsibilities

Qualification / Experience
- Valid Georgia ARNP license.
- Reliable transportation and current state driver's license and proof of auto insurance.
- Current DEA license.
