Job Summary :
The PASRR Specialized Services Staff / MPW Assessor / Waiver CM is responsible for conducting Initial MPW Assessments, and Reassessments for outside CM agencies per the State Contract for our 7 county coverage area.
This position is also responsible for providing 1 : 1 specialized services to those who live in nursing facilities and have been approved by the State to receive these services.
May carry a small caseload for those participants receiving waiver services.
Minimum Qualifications :
- High School Diploma / GED
- 1 year experience working with individuals with intellectual and / or developmental disabilities.
