Case Manager
HealthCare Support is actively seeking a Case Manager to fill an opening in Gainesville, FL.
Daily Responsibilities for the Case Manager :
- Assesses patients' medical and social needs to provide the right care.
- Leads communication and care planning between doctors, patients, families, and hospital teams.
- Ensures care is efficient and meets quality standards while coordinating with insurance and outside agencies.
- Monday-Friday Day shift!
Required Qualifications for the Case Manager :
- Clear and Active FL RN License
- Minimum 3+ years of Acute Care experience
- Case Manager experience preferred

Pay Details :
- $32- $48+ an hour (can increase dependent on experience)
