What You Can Expect :
- Works with the patient, physician and care team, to create and implement a care plan that addresses the identified needs
- Will serve as the primary contact, advocate and resource for patients, families and physicians
- Assesses patient's knowledge of their clinical condition(s) and provide education and self-management support based on the patient's unique learning style
- Will measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served
- Performs ongoing education to patient and patient's family and / or caregiver on diagnosis, prognosis, treatment and medications
- Full Time with Benefits and hospital contributions
- Monday-Friday with flexibility in remote or on site work
Qualifications :
- Education : Masters Degree
- Experience : 2 years experience
- Licenses / Certifications : Licensed Master Social Worker (LMSW) in Idaho
