JOB SUMMARY
RESPONSIBILITIES :
- Coordinates operations of a community navigation program that bridges inpatient care to patient care in the community.
- Tracks and reports project outcomes and represents the project within The Queen’s Medical Center (QMC) and in the community.
- Ensures the achievement of optimal outcomes through superior customer service, appropriate use of resources, and interdisciplinary collaboration.
- Performs functions related to enhancing the physical, emotional and social functioning of patients recently discharged from QMC.
II. TYPICAL PHYSICAL DEMANDS : All are essential designated by frequency.
- Essential : seeing, hearing, speaking, static gripping of an object for prolonged periods.
- Frequent : standing, sitting, walking, lifting and carrying usual weight of 2 pounds, reaching above, at and below shoulder level, repetitive arm / hand motions.
- Occasional : stooping / bending, kneeling, climbing stairs, walking on uneven ground, squatting, twisting body, finger dexterity, frequent gripping of an object.
- Operates computer, telephone, copier, facsimile, typewriter; cellular, pager, and other office type equipment.
III. TYPICAL WORKING CONDITIONS :
- Not substantially subjected to adverse environmental conditions.
- Exposed to communicable diseases and bodily fluids.
- May be subjected to emotionally charged situations which may compromise physical safety.
IV. MINIMUM QUALIFICATIONS :
A. EDUCATION / CERTIFICATION AND LICENSURE
- Master’s degree in Social Work.
- Current Hawaii state licensure as a Licensed Social Worker (LSW).
- Current Hawaii state licensure as a Licensed Clinical Social Worker (LCSW) preferred.
- Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations.
- Driver's abstract or evidence of good driving record.
- Current BLS / HCP certification.
- Certification from the National Association of Social Workers in the Academy of Certified
Social Workers (ACSW) preferred.
B. EXPERIENCE :
- Two (2) years work experience as a Social Worker, preferably in a community setting / assignment.
- Prior leadership experience and / or experience to demonstrate ability to lead projects.
- Experience to demonstrate knowledge of community resources and the ability to coordinate community resources effectively.
- Working with underserved populations highly desirable.
