OverviewPay range for this position is $32.83 - $48.76 / hour. Actual pay is based on years of licensure.POSITION SUMMARY : The ED RN Case Manager is responsible for identifying, assessing, planning and coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load.
Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services.
The Case Manager assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation.
Assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with hospitalizations and transition plans.
Coordinates with the health care team to facilitate and expedite the best outcomes for the patient and physician caring for that patient.
The Case Manager works with the patient and treatment team to help establish goals and realize outcomes as determined by current individualized care plans.
Works with the Care Management Division leadership team to ensure alignment with the strategic goals and visions.QualificationsREQUIRED QUALIFICATIONS : Education : Bachelor's Degree in Nursing must be obtained within 3 years of date of hire into role.
Licenses / Certifications : RN-Registered NurseExperience : 1 year of nursing experiencePREFERRED QUALIFICATIONS : Education : Bachelor's Degree in Nursing Care Management certification for adult or pediatric, as appropriate, within 3 years of hire.
Exam must be taken by 3rd anniversary. If are unable to successfully pass the certification exam, this deadline can be extended by 1 year.
Experience : 3 years of experience caring for patientsCare management experience with chronic, complex illnesses in adult and / or pediatric populationsOther requirements / information : Broad knowledge of chronic illness and medical and community resourcesWorking knowledge of a clinical EMR system and Microsoft Office applicationsDemonstrated knowledge and skills necessary to provide services to the assigned patient populationFlexible work schedule-weekend / holiday / on call rotationProven ability to work independently and efficiently with excellent time management skillsExcellent interpersonal skills to develop and maintain cooperative working relationships with personnel form other departmentsPositive, empathetic approach and ability to communicate effectively to patients / families with a variety of health issues, age groups and socioeconomic backgroundsOSF HealthCare is an Equal Opportunity Employer.
