Job Summary :
Rosalie’s Place is a transitional housing facility that supports working or ready-to-work adults with access employment and temporary bridge from homelessness to permanent homes.
Reporting to the Program Manager, case managers assist transitional housing residents in accessing care and services, advocating for resident needs, and assist in removing barriers to accessing gainful employment and permanent housing for residents.
The case manager works with assigned residents to develop employment and housing plans and assists residents toward income and housing goals.
The ideal candidate enjoys working as part of a team of staff and volunteers and can think and act quickly when there is a need to pivot or address an emergency.
This candidate can think critically and help guests determine a course of action that will move them toward stability. This individual can work independently while ensuring complete and thorough communication with team members.
The best candidate for this post is very organized, detail oriented, comfortable with creating lines of communication with community programs, and able to encourage and support follow through from residents.
This representative must be energetic, resourceful, and reliable with strong interpersonal skills and a passion for working with vulnerable populations.
This person will exhibit the demonstrated ability and desire to maintain and promote an organized, safe, and efficient work environment.
The goal of this position is to provide program residents with the tools and support required to permanently end their homelessness.
Specific Duties :
- Carries caseload of up to 25 residents, meeting with each resident a minimum of once a week.
- Completes intake appointments with prospective residents, including gathering background information and writing intake assessments.
- Develop and monitor individualized service plans (ISPs) in collaboration with residents; focus on skills development, job readiness, budget and savings planning, and career and housing goals.
- Provide counseling, guidance, and support to clients facing barriers to employment.
- Coordinate with employers, training providers, and community organizations to facilitate job placement assistance and training opportunities.
- Transport guests to appointments, as needed.
- Maintains timely and accurate records in case file and HMIS, including contacts with resident, family members and social agencies, etc.
- Participates in weekly case conferencing to monitor resident progress, problem solve with peers, and share resources.
- Maintains timely and thorough case notes and completes monthly reports for program manager.
- Coordinates with other program staff as needed regarding resident interaction within the program.
- Assists resident with navigating service delivery systems in the community, including attending appointments as needed and providing hands on assistance to overcoming barriers.
- Coordinates and advocates with referral agencies to facilitate appropriate services.
- Mediates and facilitates problem solving with residents, including conducting group meetings or individual coaching.
- Other duties as assigned by the program manager.

Required Skills / Abilities :
- Excellent communication, organization skills, and ability to interact with guests, staff, and volunteers required.
- Related experience working with vulnerable populations in a transitional housing or shelter environment preferred.
- Knowledge of available community support and services is beneficial and will be shown preference.
- Strong computer skill, proficiency with Microsoft Suite programs, and excellent electronic communication and documentation preferred.
- Must have valid driver’s license with a clean driving record.
- Must have or be able to obtain a Level One Fingerprint Clearence Card within 90 days of being hired.
Education and Experience :
- BA / BS in social work or related field preferred, or equivalent experience
- Proficient in standard computer systems (Microsoft Office)
Physical Requirements :
- Abilities to stand and walk, lift up to 50 pounds, sit, bend, and reach are required for this position.
- Ability to read, comprehend, and communicate orally and in writing required.
