CarleHealth is seeking an additional Advanced Practice Provider to join our Family Medicine Team in Metamora, Illinois.
Practice opportunity details include :
- Float position that will be based out of our Metamora clinic but will provide coverage at our different Family Medicine and Walk-In clinics in the Greater Peoria region.
- Work with established teams consisting of Physicians and other experienced APPs.
- Experienced staff, both clinical and non-clinical who know the patients extremely well.
- X-ray and lab services available onsite at most locations.
- Average patients / day : 12-16.
- Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination.
- No call requirements.

Education Requirements :
- Master's degree in Nursing or Physician Assistant.
- Family Practice experience strongly preferred.
Licensure / Certification Requirements :
Current Illinois registered nurse and advanced practice nurse / physician assistant licensure.
