The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility's Social Services Department.
As a Social Worker, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
- Assist in planning, developing, organizing, implementing, evaluating, and directing the facility's social service programs.
- Participate in discharge planning, resident assessments, and developing social care plans.
- Interview residents / families to obtain social history.
- Refer residents / families to appropriate community agencies when needed.
- Provide consultation to staff and community agencies on meeting residents' social / emotional needs. Interpret residents' social, psychological, and emotional needs to the care team.
- Assist in obtaining resources from community agencies to meet residents' needs.
- Ensure confidentiality of residents' protected health information.
The job requires a Bachelor's degree in Social Work, at least 1 year of supervised experience in healthcare, and Massachusetts Licensure Now that you have spent the last few minutes reading about this role, you may as well apply.
