Notre Dame Health Care Center's Hospice and Palliative Care Team seeks a compassionate and experienced Licensed Clinical Social Worker ( LCSW or LICSW ) to join our dedicated Hospice Care team.
In this vital role, you will provide invaluable support and counseling services to patients and their families during end-of-life care, helping them navigate the emotional challenges associated with this stage of life.
Your expertise in clinical social work, coupled with a deep understanding of the psychosocial aspects of terminal illness, will make a significant difference in the quality of care and comfort we offer.

- Conduct comprehensive assessments of patients' social, emotional, environmental, financial, and support needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team
- Provide counseling and psychotherapy to patients and their families to help them cope with end-of-life issues
- Educate patients and families about hospice care, advanced directives, and other relevant topics
- Facilitate family meetings to discuss goals of care, patient wishes, and to resolve conflicts
- Coordinate with community resources to ensure patient and family access to supportive services
- Advocate for patients' rights and self-determination in accordance with their wishes
- Document patient assessments, interventions, and progress in the electronic health record
- Collaborate with medical and professional staff to ensure a holistic approach to care
- Support the bereavement team in providing grief counseling and support services to families after the patient's death
- Participate in ongoing professional development and in-service training to stay current with best practices in hospice care
- Participate in quality assurance and performance improvement activities
PM17Requirements :
- Master's Degree in Social Work (MSW) from an accredited school of social work
- Valid and current state licensure as a Clinical Social Worker (LCSW)
- Minimum of two years of clinical social work experience, preferably in hospice, palliative care, or a related health care setting
- Knowledge of psychosocial and emotional aspects of terminal illness and bereavement
- Experience in conducting comprehensive psychosocial assessments and developing individualized care plans
- Proficiency in providing counseling and support to individuals, families, and groups
- Ability to collaborate effectively with an interdisciplinary team
- Excellent communication and interpersonal skills, both verbal and written
- Strong organizational and time management skills
- Comfortable with home visits and providing end-of-life care in various settings (patient's home, nursing facilities, etc.)
- CPR certification preferred
- Must have a valid driver's license and reliable transportation
- Availability for on-call rotation to address urgent patient and family needs outside of standard office hours
