Job Description :

As the CNP at Kingston Healthcare, you'll be responsible for providing excellent primary care and specialized healthcare services to our amazing residents.

You'll work hand-in-hand with the rest of our healthcare crew to ensure that our residents receive top-notch, patient-centered care every day.

You'll diagnose, treat, and manage medical conditions, promote wellness, and uphold the highest standards of clinical practice.

Responsibilities :

• Do a thorough check on our residents' health by conducting physical exams, medical history reviews, and diagnostic tests to come up with accurate diagnoses and personalized care plans.
• Manage and address acute and chronic medical conditions by prescribing medications and therapies that work best for each patient.

You'll also be responsible for monitoring their response to treatment.

• Educate residents and their families about how to maintain their health, prevent illness, and take care of themselves - they're an important part of the team too!
• Work with the whole healthcare squad, which includes doctors, nurses, therapists, and other healthcare professionals, to coordinate care, resolve complex health issues, and ensure that residents receive the best possible care.
• Keep accurate and timely records of patient encounters, assessments, treatment plans, and progress notes adhering to regulatory standards and best practices.
• Stay up-to-date with quality improvement initiatives, evidence-based practice, and continuing education to enhance clinical knowledge and contribute to the evolution of healthcare delivery at Kingston Healthcare.

Qualifications :

• You'll need a Master's or Doctoral degree in Nursing with a specialization as a Nurse Practitioner from an accredited program.
• You must be National Nurse Practitioner certified, with a specialization in adult-gerontology, family, or another related area of practice.
• You need an active state Nurse Practitioner license and should be able to practice independently and prescribe medications.
• Wound Care Certified (WCC) or wound care experience preferred.
• Previous experience working in primary care, geriatrics, or long-term care is preferred, but if you have relevant clinical rotations, that's okay too!
• Great clinical assessment and diagnostic skills are a must. You'll also need to be able to manage a diverse range of acute and chronic medical conditions in older adult populations.
• You'll need to have excellent communication, interpersonal, and teamwork skills and approach your patients with compassion and a patient-centered attitude.
