About the Organization
Community Physical was founded in 1986 by Bob Tripicchio, PT, MS, DSc, on the premise that people are the most important asset of any organization.
CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services.
Throughout this growth period, CPT has remained focused on maintaining a practice that creates values for employees so as to leverage the best possible knowledge possible.

Our team of board-certified therapists offers Physical, Occupational, and Speech Therapy in post-acute rehabilitation, skilled nursing facilities, home settings, and outpatient locations.

Job Responsibilities :
Provide therapy services in alignment with CPT's policies and the goals of the facility.
Maintain rehabilitation standards and perform administrative duties essential to the rehab department's daily operations.
Develop strong relationships with patients, family members, third-party payers, and others involved in patient care.
Qualifications :
Master's degree in communication disorders.
Must meet the requirements for the State of Illinois Department of Professional Regulation or the State of Wisconsin, as applicable.
Certificate of Clinical Competence (CCC) from the American Speech and Hearing Association (ASHA) or actively completing certification in Clinical Fellowship Year (CFY).
Skilled Nursing Facility (SNF) experience preferred.
Knowledge of Net Health is a plus (training available).
