Responsibilities : Under the supervision of the Mason Rural Outreach Program Coordinator, will provide nursing services for adults with severe and persistent mental illness.
Will function as a member of the interdisciplinary team. Nursing duties may include completing health assessments, providing referrals to community agencies, assisting in linking to primary care physicians, administering prescription medications, and coordinating with hospitals, medical specialists, and treatment teams.
May render general nursing care and provide medical support following approved nursing practices. Responsible for extensive case documentation.
Additional responsibilities may include but are not limited to, providing information and assistance specific to the area of assignment, performing admissions screening on new consumers, serving as agency liaison, performing crisis intervention activities, providing patient education, and potentially providing the above services on an outreach basis.
Responsible for carrying out all activities of the program / sub-unit in such a manner that fulfills CMHA’s mission, policies, and procedures.
Requirements : Possession of an Associate’s degree or a diploma in Nursing is required. Possession of a State of Michigan license as a Registered Nurse is required.
Possession of Basic Life Support for Healthcare Providers Certification required. Case Management duties may only be assigned when the employee has or attains one year of experience.
Possession of a valid Michigan driver’s license and reliable transportation for job related use required.
Conditional Employment Requirements : Employment offers for this position will be contingent upon the results of criminal background verification, credentialing procedure consisting of Michigan State Police Clearance Check, Verification of Health Professional License, Medicaid / Medicare verification, verification of acceptable driving record, educational background check and verification of acceptable work history.
Must be able to pass a pre-employment 5-panel drug screen.

Salary / Hours : $28.74 - $34.30 per hour. Monday Friday 8 : 00 am 5 : 00 pm some evening hours may be required
