A company is looking for a Palliative Care Nurse Practitioner.Key ResponsibilitiesConduct specialty palliative care outpatient appointments via video visits as part of an interdisciplinary teamOversee interdisciplinary case conferences to create and monitor individualized care plans for members with advanced serious illnessProvide education on palliative care to Community Health Workers and Oncology Nurses, focusing on symptom management and advance care planningRequired Qualifications, Training, and EducationAt least 3 years of nurse practitioner experience, with a minimum of 2 years in palliative careAdvanced certified hospice and palliative nurse practitioner (ACHPN) certificationUnrestricted nurse practitioner license with a willingness to obtain additional state licensesExperience in an outpatient oncology setting is preferredFamiliarity with technology and virtual care platforms
