About The Position
Job Title : Physical Therapy Assistant Job Specialty : Rehabilitation Therapy Job Duration : 13 weeks Shift : Monday to Friday, Days Guaranteed Hours : 32 hours per week Experience : New graduates are welcome, Skilled Nursing Facility (SNF) experience preferred License : State License Verification required Certifications : None specified Must-Have : Fill out and attach the prescreen form upon application- Attach resume and license verification Job Description : Ensure all required documentation, including prescreen, resume, and license verification, is submitted;
incomplete applications will not be considered.- Conduct rehabilitation therapy under the supervision of a physical therapist.
- Assist patients in exercises and therapeutic practices to improve mobility and function.- Conduct assessments and implement personalized care plans.
- Collect patient data and provide updates on progress to the supervising therapist.- Conduct a COVID-19 test 72 hours prior to start date with results issued 48 hours prior.
- Complete credentialing checklist which includes : - State License Verification - Resume and three references - County, Federal, and Multi-State Criminal Search (last 7 years) - Nationwide Sex Offender Search (last 7 years) - HHS / OIG and GSA Search - Social Security Number Trace - 1 Step PPD, Quantiferon, or proof of positive PPD and chest X-ray - SAM and DOJC SearchThis position does not require holiday coverage.
Bilingual skills are not required.
