Eager to partner with highly motivated RN (Registered Nurse) with strong clinical and assessment skills to oversee patient hospice care.
- Interviews are being scheduled for RN (Registered Nurse) Case Manager for the Kansas City Metro service area! This position will be based in MO.
- Hospice experience is preferred.
This is a Full-time position with the opportunity for bonus visit pay . Mileage reimbursement. Holiday and Paid Time Off, Health Insurance option, Dental, Vision, and 401K Benefit opportunity.
The Registered Nurse Case Manager plans, organizes and directs hospice care services and is experienced in nursing. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families.
Patient care is provided in the patient's home environment and family members and caregivers are supported with education and on going access to community resources.
Must be a licensed Registered Nurse in both MO and KS.
Must be willing to travel throughout the KC Metro.
