Status : Full-time, Non-Exempt (Hourly), Non-Management

Location : Santa Monica Old Firehouse, 1444 7th St, Santa Monica, CA 90401

Schedule : Monday- Friday 8 : 00am- 4 : 30pm

Summary

With the support of and in partnership with the Santa Monica C3 Program Manager and Westside Outreach leadership, the Senior Clinical Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in high density areas of Santa Monica.

The Senior Clinical Case Manager is responsible for identifying, engaging and providing support to homeless clients, including providing referrals, linkages, and coordinating care.

The Senior Clinical Case Manager provides information and support for co-workers, and assists in training and mentoring in linking to a variety of resources and services as needed.

The Senior Clinical Case Manager will provide risk assessment and high level clinical interventions and assessments, assisting clients and the team in attaining wellness and safety.

Essential Duties and Responsibilities

  • Provide field-based services, meeting currently homeless clients where they are in order to eliminate barriers to services.
  • Assess mental health service needs of clients.
  • Engage clients in developing mutually agreed-upon goals documented in each client’s confidential service plan.
  • Provide risk assessment and clinical evaluations for clients who are potentially gravely disabled and in need of hospitalization and referral to higher levels of care.

Ability to initiate or be trained to initiate psychiatric holds.

  • Link clients with appropriate medical, mental health, substance abuse, and vocational resources.
  • Assist clients in obtaining all benefits to which they are entitled, including applying for and advocating for SSI benefits.
  • Provide opportunities for clients to build a supportive peer community.
  • Build relationships with County mental health and other resources to increase collaboration and responsiveness to clients with severe mental illness and addiction.
  • Complete all charting in a timely manner.
  • Collaborate and communicate effectively with police officers and local hospital staff to reduce police contact and ED visits by targeted population.
  • Work effectively on an interdisciplinary team and collaborate with Program Manager to ensure high level of clinical oversight.
  • Other duties as assigned.

Qualifications

  • Experienced Mental Health (Licensed or near-licensed) professional (LCSW, LMFT, PhD, PsyD) with capacity and willingness to get LPS designated for initiating psychiatric holds.
  • Demonstrated knowledge of clinical case management.
  • Experience in dealing with issues faced by population served (serious mental illness, substance abuse, and physical health problems).
  • Excellent writing and charting skills.
  • Knowledge of community resources.
  • Able to maintain an effective working relationship with Santa Monica community partners (including police, firefighters, city staff, etc.

and maintain and build professional rapport.

  • Ability to self-motivate, work independently, and communicate effectively.
  • Able to work collaboratively as part of an interdisciplinary team.
  • Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.

Preferred Qualifications

Experience in mental health, substance abuse, medical setting.
