Job Description :

Supplemental Health Care is currently hiring LPNs with previous OASIS experience for home health contracts in Fort Wayne, Indiana, and the surrounding areas.

Home Health Nurse Contract Details :

• $1, - $1, per week*
• SHC Home Health Nurses are paid by the hour, not by the visit
• LPN will have a designated territory of coverage
• Weekly expectation is 30 units of productivity
• Home Health contracts with SHC require previous experience with OASIS / Medicare Certified Visits
• Estimated weekly payment may include both taxable wages and tax-free reimbursements for meals, housing, and incidentals.

Requirements :

• OASIS / Medicare Certified Visits Experience
• Electronic Charting experience
• Minimum of 1 year recent home health nursing experience (private duty does not qualify)
• Most contracts require some on-call
• Current Indiana Nursing License
• Valid U.S. Driver's License
