Responsibilities
The Clinical Liver Transplant Coordinator (RN) will be responsible for :
- Provide education and assessments for potential liver transplant recipients
- Expedite necessary care and treatment using critical reasoning, particularly for patients with complex or life-threatening conditions
- Collaborate within the multidisciplinary healthcare team to evaluate and manage liver transplant patients
- Present patients at the Liver Selection Committee
- Submit exceptions for waitlisted patients as needed
- Take a semi-autonomous, self-directed role in accordance with professional practice standards and clinical expertise
- Coordinate patient communication, education, and the management of liver transplant patients throughout the continuum of care
- Integrate quality assessment, performance improvement, clinical research, and teaching to enhance patient care activities
Qualifications :
- 4+ years of Clinical experience in a highly specialized complex Care program
- Bachelor's Degree in Nursing
- NYS Registered Nurse (RN) license
- Spanish speaking
Desired Skills :
- Master's Degree in Nursing
- Transplant experience
