Case ManagementFull TimeDays w / Call8am-430pm with callReq# 9273Responsible for direct social work case management services to the participants of the StayWell Senior Care program.
Provides social services support as : participant screening, case management, counseling and referral.Works closely with participants and their families to ensure adjustment to illness by addressing both psychosocial and practical needs of participants.
Services consist of assessment, diagnosis, treatment planning, psychotherapy, and counseling for individuals, couples, families, and groups.
Also responsible for crisis assessment and intervention.Must demonstrate knowledge of best practice in screening and treatment for participants at risk of suicide and for distress screening and management.
Demonstrates knowledge of, and ability to interact with, persons with psychiatric disorders.Applies social work theory and methods for the treatment and prevention of psychosocial dysfunction, disability or impairment, including emotional and mental disorders.
Master's Degree from an accredited School of Social Work.LCSWA (Licensed Clinical Social Worker Associate) preferred or examination must be completed within the first year of employment.
Possess individual therapy skills for geriatric participants, analytical thinking and problem solving abilities. Willing to conduct home visits to private homes of participants in Randolph, Montgomery and Moore Counties.
Only act within the scope of his / her authority to practice.Meet a standardized set of competencies for the specific position description established by StayWell Senior Care and approved by CMS before working independently.
Be medically cleared for communicable diseases and have all immunizations up-to-date.Have valid NCDL and vehicle.Two years experience in a health related area.
Minimum of one year experience working with the frail elderly population.
