Registered Nurse

Positions Available :

RN Case Manager (Homecare)

This position is a full-time position working 40 hours a week. The typical hours are 8 : 00 am - 4 : 30 pm Monday through Friday, with a weekend and holiday rotation.

The nurses typically work every 4th or 5th weekend and holiday.

RN Care Center - PT Day

This position is a PT, 12-hour position, 6 am- 630 pm, every third weekend. One shift per week, but opportunity to pick up more.

RN Care Center - PT Night

This position is a PT, 12-hour position, 6 pm - 630 am, every third weekend. One shift per week, but opportunity to pick up more.

Responsibilities :

• Conduct assessments of patient symptoms and administer appropriate advanced care management.
• Organize interdisciplinary patient meetings to ensure seamless continuity of care.
• Provide supervision to staff members, including volunteers, hospice aides, and primary caregivers.
• Educate patients and their families on insurance benefits.
• Obtain essential medical orders from Physicians and Medical Directors.
• Perform initial nursing assessments and arrange for necessary medical equipment.
• Stay updated on job-related procedures and requirements.
• Carry out any additional tasks as assigned.

Qualifications :

• Must have graduated from an approved School of Nursing.
• Should hold current RN licensure in the State of Ohio and be eligible to obtain licensure in the State of Indiana, or vice versa.
• Requires a minimum of two years of experience in Medical / Surgical, Oncology, or community health nursing.
• Preferably, should have experience in hospice care.
