Job Title : Medical Social Worker
Department : Care Management
Status : Full-Time, Day Shift (M-F 9A-5P with weekend rotation)
Salary Range : $88,000 to $130,000
Sign On Bonus : $5,000
About the Role :
As a Medical Social Worker you'll play a vital role in providing comprehensive psychosocial support to patients and their families.
You'll work collaboratively with a multidisciplinary team to develop and implement individualized treatment plans, addressing the emotional, social, and practical needs of patients facing complex medical challenges.
Key Responsibilities :
- Conduct comprehensive psychosocial assessments to identify patient needs.
- Develop and implement individualized treatment plans.
- Provide counseling, crisis intervention, and grief support.
- Coordinate with community agencies to ensure continuity of care.
- Advocate for patients' rights and interests.
- Contribute to the development and delivery of educational programs.
- Maintain accurate and detailed documentation.
Qualifications :
- Master's degree in Social Work from an accredited program.
- Licensed Social Worker (LMSW / LCSW) in New York State.
- Minimum of two years of experience in an acute care hospital setting or related field.
- Strong clinical skills and ability to work effectively in a fast-paced environment.
- Excellent communication and interpersonal skills.
- Passion for helping others and making a positive impact on their lives.
