Position Overview : Nurse Practitioner - Health Risk Assessments
As a Nurse Practitioner, you will perform comprehensive health risk assessments for Medicare Advantage members in their homes.
This position focuses on evaluating patients' physical, mental, and social needs without the responsibility for ongoing care.
Responsibilities of the Nurse Practitioner - Health Risk Assessments :
- Willingness to see patients in the St. Louis, MO area.
- Conduct comprehensive in-home health risk assessments on Medicare Advantage members
- Identify and document all current and chronic health conditions, assessing physical, mental, and social needs.
- Provide patient education and counseling on identified conditions and screening procedures
- Record assessment findings using a provided iPad - no prescriptions, follow-ups, or on-call duties required.
Requirements of the Nurse Practitioner - Health Risk Assessments :
- Active and unrestricted Nurse Practitioner license.
- Minimum of 3 years of patient care experience (preferably in primary care, adult or geriatric settings and with EMR proficiency).
- Minimum commitment of 18 hours per week, including one full 8-hour day (Sunday-Saturday).
- Ability to work independently and autonomously.
- Strong empathy, compassion, and interpersonal skills.
- Excellent verbal and written communication abilities.
