Day Services Specialist

Queens Centers For Progress, a non-profit agency serving adults and children with developmental and intellectual disabilities, is seeking a Day Services Specialist for our Day Program.

Duties :

On an annual and semi-annual basis, schedules team meetings to review each assigned individual’s day service plan. Revises plan based on outcome of meeting and completes all paperwork.

On a monthly basis (quarterly basis for Geriatric services), reviews the service notes for assigned individuals to assess their response to services provided.

Summarizes the individuals’ responses by completing monthly review notes (quarterly notes for Geriatric Services). If an individual’s response warrants a change in the plan, schedules a team meeting and revises the plan.

Writes narrative notes more frequently if the need arises.

Conducts assigned groups on a daily basis by implementing the day service plan for each individual in the group. Reviews the individuals’ plans;

including the valued outcomes, medical and nursing needs and behavior characteristics. Completes paperwork related to the daily service.

For all assigned individuals, instructs and supervises the Day Service Assistants in the implementation of their plans, including service methodologies, schedules, safety and / or evacuation needs, medical and nursing needs, and behavioral characteristics.

In cooperation with the Day Service Staff, plans for and organizes program and community activities, including seasonal events.

Assists in the development of community activities and volunteer experiences.

For assigned individuals who are new to the program, assess their needs and interests and develops an initial day service plan.

Trains staff in the implementation of the plan.

Performs all other duties as assigned.

Requirements :

BA / BS Degree required

One year of experience working with individuals with developmental disabilities required. Successful candidates should have good written, communication and interpersonal skills.

Job Type : Full-time

Pay : $18.00 - $22.50 per hour
