SUMMARY / RESPONSIBILITIES : SUMMARY : The Navigator will leverage assessment and direct intervention experience to identify, plan, and offer customized, easy to use, and flexible educational and coaching support to stroke survivors and their care partners through the Kandu Health digital App during the post-acute transition to home.

Under the clinical supervision and direction of the Lead Navigator, this role will be assigned a caseload to manage independently, composed of, stroke survivors, care partners and community resources, engaging them via fully remote tech-enabled healthcare services and app-based communication channels.

The Navigator will also serve as a primary point of contact to provide information about services, resources and clinical support within the survivor’s local community.

The Navigator will support key learnings for a stroke survivor and their care partner’s transition to home and life adjustments associated with the post-stroke journey.

This role facilitates a smooth transition for continuity of care in preparation for a stroke survivor’s discharge home following an acute hospitalization.

The Navigator will provide support to an assigned group of survivors from a client hospital throughout the survivor’s 90-day post-stroke engagement with Kandu.

The Navigator will work collaboratively with a team and reports to the Lead Navigator. ESSENTIAL NON-CLINICAL DUTIES AND RESPONSIBILITIES include the following.

Other duties may be assigned on an individual basis : Demonstrates cultural sensitivity and diplomacy when working with the socioeconomic diversity of stroke survivors and care partners to quickly establish rapport, build trust and maintain ongoing relationships Builds productive internal / external working relationships to resolve mutual problems by collaborating on procedures or processes.

Developing professional expertise, applies department policies to resolve a variety of issues Receives little instruction on day-to-day work, general instructions on new assignments Possesses excellent writing skills with the ability to complete documentation within deadlines Demonstrates the ability to work efficiently on multiple work streams Takes initiative to familiarize oneself with the Kandu program including learning content for survivor and care partner support throughout the 90 days post-discharge Demonstrates resourcefulness in learning about stroke-related resources at the client hospital and in the survivor’s discharge to home location Exhibits creativity and the ability to be innovative in a fast-paced environment with a lot of ambiguity and constant change Strong organizational skills, flexibility, adaptability, and the ability to multitask required Actively promotes and supports the Quality Management System, Quality Objectives, and Quality Policy Applies all applicable procedures and processes according to Regulatory requirements Actively promotes and support the company’s Management Review process Informs responsible personnel of concerns involving clinical quality Performs job functions in a safe and effective manner Other duties as assigned.

ESSENTIAL CLINICAL DUTIES AND RESPONSIBILITIES : Clinician shall perform the duties outlined below and other duties as assigned by Kandu Medical Services West PC (KMS) and Kandu Medical Services East, chartered, within Clinician’s scope of licensure and training.

This practice will be in accordance with the highest degree of professional competence and meeting the highest ethical standards of the profession, and further perform all such duties in accordance with KMS’ guidelines.

Clinicians shall comply with the terms and conditions of all contracts respecting medical services to be provided by KMS to contracting third-party clients and customers, and with the rules, regulations and procedures and the usual and customary duties of the clinical staff of any such contracting third parties.

Clinician serves in the role of Navigator and will report to the Lead Navigator Escalates clinical concerns to the Lead Navigator for review and troubleshooting Provides direct support to stroke survivors and their families to efficiently manage a caseload of up to 40 stroke survivors engaging with them via fully remote, telephonic, and app-based communication channels Demonstrates active listening and communication skills necessary to effectively coach survivors and their care partners Ascertains educational needs and unmet social needs of survivors and care partners through continuous collaboration with the clinical care team, survivors, and care partners.

Facilitates a smooth transition for continuity of care in preparation for a stroke survivor’s discharge home following an acute hospitalization Available to take on survivor escalation calls (i.

e. report of suicidal ideation, etc.) during non-work hours, weekends and holidays, as needed Leverages assessment and direct intervention experience to identify, plan, and offer personalized educational and coaching support to stroke survivors and their care partners through the Kandu Health App during the post-acute transition to home Supports key learnings for a stroke survivor and their care partner’s transition to home and life adjustments associated with their post-stroke recovery.

This includes, but is not limited to : Conducting onboarding interviews, deploys assessments, and facilitates virtual support groups for stroke survivors and care partners Strategically deploying education and connecting clients with resources based upon case analysis Conducting social needs assessment and deploying related resources to improve transition to home and management of risk factors for readmission Performing remote home safety assessments and providing related recommendations or working with another clinical team member to ensure home safety assessment completion Completing ergonomic evaluations and recommendations for return-to-work and other activities Conducting health-related and quality of life assessments and providing related clinical recommendations Strategies for socialization and engagement in exercise programming Cognitive and behavioral health assessment and recommendations per scope of practice Recommendations for use of and training on adaptive equipment Use of lifestyle medicine principles and lifestyle management such as medication management strategies, health behavior change, healthy meal preparation given survivor impairments, etc.

Facilitation of the Take Charge intervention for purposes of survivor planning and goal setting Psychosocial assessment and management Provide support and outreach services to stroke survivors and care partners in the event of a critical incident (suicidality, abuse, neglect, etc.

Participation in multidisciplinary case conference meetings to discuss and plan for survivor and / or care partner needs Salary Range : $62,100-79,200 annually Please note that the salary information is a general guidance only.

Kandu Health, Inc. considers factors such as scope and responsibilities of the position, candidate’s work experience, education / training, key skills and internal equity, as well as location, market and business considerations when extending an offer EDUCATION / EXPERIENCE : 2-5 years of experience in a patient-facing capacity, experience with caring for individuals post-stroke (in either personal or professional capacity) highly preferred Occupational Therapist or Licensed Clinical Social Worker Upon post hire, must be flexible to obtain licensure in other states as the customer base grows.

Experience working with ethnically, culturally and racially diverse survivors, caregivers and staff Proficiency in basic medical terminology Logistical / Technical Demonstrates the computer skills and knowledge necessary to effectively use and support various programs and digital applications Familiarity with electronic medical record technologies preferred Must have regular and reliable internet access to allow for team meetings via online platforms and online support of clients through synchronous and asynchronous means Consistent access to quiet, private area for conducting client engagement without interruption Timely follow up to stroke survivor / care partner questions within the Kandu mobile application Continuous review of assessments and screening tools within the mobile application and follow up with Lead Navigator as appropriate Preferred Qualifications Training in coaching competencies, theories and methodologies Group facilitation experience for management of stroke survivor support group cohort Proficiency in conversational Spanish (writing and speaking) preferred WORK ENVIRONMENT : This is a fully remote position This job does not offer relocation SUPERVISORY RESPONSIBILITIES : No direct supervisory responsibilities.
