Community Health Worker
Job Description :
The Community Health Worker (CHW) will work closely with providers, patients, and community partners to coordinate quality patient care with fluency in both English and Spanish.
The CHW will assist with patient scheduling, reminders, coordination of transportation for appointments and the timely completion of applications.
They will connect patients to community resources and conduct home visits if needed to provide patient education and support.
Hours per Week :
Full-time : 40 hours per week
Reports to :
Patient Access Director
General Responsibilities :
- Collaborates with other members of the clinical care team in developing patient-centered care plans.
- Assists Patient Access Director and case management / CHW team with patient case management needs such as subspecialty care appointments, medication access assistance, and Medicaid enrollment applications when applicable.
- Documents individual contacts / visits in the Electronic Health Record and outcomes data as instructed in a timely manner.
- Advocates for and supports technology-enabled patient education and care.
- Collaborates with community partners to establish and maintain on-going support options.
- Contributes to the achievement of established program goals and objectives, and adheres to department policies, procedures, and quality standards.
Experience :
Prior clinical experience may include work as receptionist, medical assistant, case manager, or social worker. Must have at least 1 year of experience in a clinical setting.
Qualifications and Education :
- Must be fluent in English and Spanish.
- Bachelor's degree or higher level of education preferred.
- Excellent verbal and written communication skills.
- Basic computer skills including use of Microsoft Office.
- Highly organized with strong attention to detail.
- Strong problem solving and critical thinking skills.
- Must be able to work with a diverse patient population and have exceptional customer service skills.
