Our team provides 1 on 1 care in patients' homes with extensive services to provide our patients with the best possible care.

Our Start of Care / Admissions Registered Nurse oversees all (home based) skilled admissions based off the plan of care for patients located within the Richmond, Glen Allen, Henrico Mineral VA area.

Essential Job Responsibilities :

  • Makes the initial start of care evaluation visit and completes and submits OASIS assessments in accordance with Agency defined time frames.
  • Initiates the plan of care and necessary revisions.
  • Counsels the patient and family / significant others in meeting nursing and related needs.
  • Participates in and presents in-service programs.
  • Understands and adheres to established Agency policies and procedures.
  • Determines the amount and type of nursing needed by each individual patient.
  • Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
  • Conducts patient care conferences on patients assigned to his / her care.
  • Participates in peer review and performance improvement as assigned.
  • Participates in utilization review of medical records as assigned.
  • Gives total patient care as needed.

Required Qualifications :

  • Graduate of an approved school of professional nursing
  • Current and valid Virginia RN License
  • Passionate and motivated to provide care within a patient's home.
  • Ability to exercise initiative and independent judgment.
  • Comfortable working collaboratively in cross-functional teams
  • Ability to communicate effectively, both verbally and in writing, with all levels of an organization
  • Ability and willingness to travel within a given territory.
  • Willingness to work with multiple patients in varying home locations.

Preferred Qualifications :

  • Two (2) years' home health nursing experience, preferred.
  • Previous experience with Home Care Home Base EMR system strongly preferred.
  • Wound Care / Wound Vac experience or Certification strongly preferred.
