Job Posting : Registered Nurse Care Manager (RNCM)
Location : Baltimore, MD
Schedule : Full-Time (40 hours per week)
- Monday : 10am-7pm
- Tuesday - Friday : 8am-4 : 30pm
Position Overview :
The Registered Nurse Care Manager (RNCM) plays a vital role in providing comprehensive care management and coordination for a dedicated panel of patients.
The RNCM serves as an advocate, educator, and resource, ensuring a holistic approach to patient care. You will collaborate closely with clinicians to develop and implement personalized care plans, addressing barriers to care including logistical, emotional, financial, and social challenges.
Working under the Clinical Manager’s direction, the RNCM facilitates patient compliance with treatment plans through regular care coordination, patient education, and follow-ups.
The RNCM may occasionally support multiple centers and teams, collaborating with nurses, staff members, and external providers to fulfill patient care needs.
Qualifications :
- Active RN License in Maryland
- 1-2 years of outpatient experience preferred (candidates from inpatient acute care or case management settings will also be considered)
- Strong coordination and communication skills, with experience in population health or care management being highly desirable
Compensation :
- Pay range : $40.00 - $45
Travel/Contract - RN-Case Manager - $2058 per week
