Description
The City of Fishers is seeking a full-time Public Health Nurse (PHN) to join our Health Department. This role is a member of the public health nursing team, which includes providing clinical services and case management support for FHD programs.
The PHN in this role will also serve as lead for one or more public health program areas including, but not limited to : Communicable Disease Case Surveillance, Emergency Preparedness, Immunizations, Maternal Child Health, and Lead Case Management.
Examples of Duties
- Serve patients by conducting home visits, when appropriate, to monitor individual and family health needs.
- Provide nursing services, apply procedures and create health care plans for patients.
- Work within the scope of community resources by referring patients to the appropriate local agencies.
- Organize logistics of health services provisions for the community and participate in discussions with various medical providers to work towards specific and positive community health outcomes.
- Educate families on the prevention of disease and making positive strides toward improved health.
- Provide educational resources on subjects such as infectious disease, immunizations, home nursing, maternity and pediatric care and basic individual and family well-being.
- Keep abreast of pediatric health standards and school health issues by attending conferences and reading professional literature.
- Stay current on general best practices related to medical care delivery including reading literature, membership in professional organizations, and participating in networking and educational events as necessary.
- Provide instruction to parents of young children regarding public health and immunizations.
- Cooperate with families to monitor recovery of ill or injured individuals and work with community resources and medical providers to improve outcomes when necessary.
- Monitor and assess medical systems by tracking progress and health results of patients.
- Recommend changes to improve the effectiveness, efficiency, or safety of basic operations as necessary.
- Maintain inventory of medical supplies, re-ordering appropriate equipment when needed, and being present to receive deliveries.
- Complete and track all appropriate paperwork and documentation, including patient records, nursing logs and other reports.
- Comply with all medical safety and legal requirements that are mandated by the hospital, clinic, city, state and nation.
- Keep patient confidentiality in accordance with health care regulations.
- Support the reputation of the public health sector by embracing changes and modifications in various nursing policies and procedures.
- Provide appropriate case management and follow up for reportable communicable diseases.
Minimum Qualifications
- Must possess an active RN licensure by the State of Indiana;
- Highly prefer bachelor’s degree in nursing but would consider an associate degree;
- Must have at least 1 year of experience working directly with patients and medical providers as a nurse;
- Must have an active Indiana Driver's License with a record that allows the ability to be insured to drive a city vehicle;
- This job requires ability to commute to multiple locations for work assignments utilizing a personal vehicle;
- Role requires the ability to communicate clearly and concisely, both orally and in writing to a variety of audiences including those who are non-native English speakers;
- Occasional evenings and weekends are required. Typically, Nurses are expected to work one evening shift per week (to 7 : 30pm) and one weekend shift per month (Saturday 8am - 1pm).
