Key ResponsibilitiesEvaluate member needs and barriers to care, recommending personalized care plansCoordinate with providers and community resources to address unmet needs and ensure timely access to servicesMonitor member progress and update care plans as necessary, providing education to members and familiesRequired QualificationsDegree from an Accredited School of Nursing or a Bachelor's degree in Nursing2 - 6 years of related experience in nursing or care managementRegistered Nurse (RN) state licensure required;
