Description

In this role, y ou’ll evaluate and treat patients by following an individualized plan of care following an evaluation and d emonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.

Qualifications

Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.

Experience with administering standardized speech / language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
