Salary : $22.15 - $24.10 per hour, DOE.

Location : Wilmar, MN

Position Summary :

The Case Manager in Housing Stability will work directly with clients who are experiencing problems with housing to help them obtain and maintain permanent housing.

This position includes screening clients' requirements to be eligible for the program, creating tailored housing goals, and collaborating with numerous community resources and landlords.

The ideal applicant is empathic, organized, and skilled at problem solving in high-pressure situations.

Essential Functions :

  • Conduct comprehensive assessments of clients' housing needs and barriers to stability.
  • Develop and implement personalized case plans that promote long-term housing stability such as finding jobs for clients to be self sufficient
  • Coordinate with landlords, housing authorities, and other stakeholders to secure appropriate housing options for clients.
  • Ability to use the HMIS system and enter the client's information into correctly and accurately
  • Provide ongoing support and follow-up to ensure clients remain stably housed.
  • Maintain accurate and confidential client records and report on progress.
  • Advocate for clients and facilitate access to additional services such as mental health services, and employment support.
  • Participate in community outreach and education initiatives related to housing stability.
  • Provide clients with education and support on tenant rights and responsibilities.

Key Responsibilities

  • Case Management : Provide comprehensive case management services to BIPOC and low-income youth and Families, including assessment, service planning, and ongoing support to help clients achieve stable housing.
  • Client Support : Develop individualized housing stability plans with clients, addressing their unique needs and barriers.

Offer resources and referrals to community services such as education, employment, healthcare, and mental health support.

  • Advocacy : Advocate on behalf of clients to secure necessary services and support from community agencies, housing providers, and other stakeholders.
  • Documentation and Reporting : Maintain accurate and timely documentation of client interactions, progress, and outcomes in the HMIS system.

Prepare and submit reports in accordance with FHPAP and Minnesota Housing requirements.

Collaboration : Work collaboratively with other case managers, social workers, and community partners to ensure a coordinated approach to service delivery.

Participate in team meetings, case conferences, and professional development opportunities.

  • Outreach : Conduct outreach to identify and engage BIPOC and low-income youth who may benefit from FHPAP services. Build and maintain relationships with community organizations and service providers.
  • Cultural Competency : Demonstrate cultural competency and sensitivity in all interactions with clients, recognizing and respecting diverse backgrounds and experiences.
  • HMIS System Use : Efficiently use the HMIS system for tracking client data, service delivery, and reporting purposes. Ensure data integrity and confidentiality in compliance with federal and state regulations.
  • Assist clients in completing housing applications and securing necessary documentation
  • Provide clients with education and support on tenant rights and responsibilities.

Qualifications

  • Education : a bachelor's degree in social work, Human Services, or a related field. Master's degree preferred.
  • Experience : Minimum of 2 years of experience in case management, housing services, or related fields. Experience working with BIPOC and low-income populations is highly desirable.
  • Skills : Strong interpersonal and communication skills, ability to work independently and as part of a team, excellent organizational skills, and proficiency in using case management software, HMIS, and Microsoft Office Suite.
  • Knowledge : Understanding of housing resources, community services, and the unique challenges faced by BIPOC and low-income youth.

Familiarity with FHPAP and Minnesota Housing programs is a plus.

Language : Bilingual skills are an asset.

Work Environment

  • Location : Based in Faribault, Minnesota, with travel to various community locations as needed.
  • Hours : Part-time position, with occasional evenings and weekends required.
  • Salary : Competitive wages commensurate with experience, along with a comprehensive benefits package for full time positions
  • This position may require occasional evening and weekend hours to meet client needs.
  • Some travel within the Faribault metro area may be required.
  • The role will primarily be based in the organization's office in Faribault with potential for outreach in the community as needed.
