RN Outpatient - Dual Role for Primary Care/Geriatric Support - Fox Chapel/West Penn - Full Time

GENERAL OVERVIEW :

The position is a dual role. The candidate will be working on site 20 hours with Primary Care and 20 hours with Geriatric each week.

The anticipated schedule will be Monday / Wednesday and ½ Day Fridays with Primary Care and Tuesday / Thursday and ½ Day Fridays with Geriatric.

For the Geriatric role, the applicant would need to perform bedside assessments (at West Penn), so any applicants should anticipate commuting to West Penn 2.

5 days per week. May also need to attend meetings at AGH on occasion.

Additional Geriatric support consists of :

Assess patients via Epic for Elderlife Programs

Program is for patients 70 and older

Assessing for risk of dementia

Program coordinators would help with specifics of the program

On-going monitoring of patients in the program

The Registered Nurse assesses human responses and plans, implements and evaluates nursing care for individuals or families for whom the nurse is responsible.

The Registered Nurse is fully responsible for all actions as a licensed nurse and is accountable to patients for the quality of care delivered.

OF Nursing care is delivered in an outpatient setting

• Assigns or delegates tasks based on the needs of the patient and skill level of the RN, LPN or support staff. Assigns duties and lunches for staff when performing charge role.

• Supervises delegates and evaluates assigned LPN scope of practice as appropriate.
• Utilizes the nursing process to assess, plan, evaluate and implement a patient plan of care according to the individual needs of the patient as prescribed by physician, nurse and hospital policy including patient and family.
• Derives culturally and age appropriate expected outcomes involving the patient, family and other health care providers.
• Analyzes the health status of the individuals and families comparing the data with the norm, when possible, to determine patient care needs.
• Assesses patient reports / records change of condition and notifies physician and / or supervisor and intervenes appropriately and documents accordingly.
• Accepts assignments in areas other than primary unit, and performs duties within individualized competency.
• Demonstrates a commitment to continuous lifelong learning and professional development for self and others.
• Promotes advancement of the profession through participation in professional organizations.
• Seeks both formal and independent learning activities using current research findings and other evidence to maintain and develop clinical and professional skills and knowledge.
• Participates in systematic peer review and obtains informal feedback regarding one’s own practice from patients, peers, professional colleagues and others.

Identifies areas of strength, as well as, areas in which professional development would be beneficial.

• Meets time and attendance guidelines per hospital policy.

• Utilizes effective time management skills to complete assigned duties and responsibilities within established work shift.

QUALIFICATIONS : Minimum

Minimum

• Completed RN training program and have PA RN licensure or have applied for practice permit
• CPR Certification

Preferred

Certification in area of specialty
