By joining our hospice team, you will play a crucial role in enhancing the lives of seniors and their families, making a meaningful difference with every interaction. Responsibilities
- Provide compassionate care focused on comfort and quality of life for patients nearing the end of life.
- Develop, implement, and manage individualized patient care plans under hospice protocols.
- Conduct regular patient visits according to the established schedule, assessing patient needs and adjusting care plans.
- Educate patients and families on end-of-life care, disease progression, and available resources.
- Coordinate with interdisciplinary hospice team members, including social workers, chaplains, and therapists, to deliver comprehensive patient-centered care.
- Administer medications and treatments as prescribed, monitoring for effectiveness and managing symptoms.
- Document patient progress, changes in condition, and any interventions in the medical record promptly and accurately.
- Communicate proactively with patients, families, and the care team to address concerns and provide emotional support.
- Participate in hospice team meetings and contribute to developing patient care strategies.
- Comply with all state, federal, and regulatory guidelines related to hospice care.
Requirements
- Active and valid RN license in the state of Ohio.
- Minimum of two years of nursing experience required.
- Prior experience in Med / Surg, Home Health, or Hospice settings is preferred.
- Demonstrated understanding of hospice philosophy and end-of-life care.
- Strong communication skills and ability to engage effectively with patients, families, and colleagues.
- Valid Ohio driver’s license and reliable transportation for patient visits.
- Ability to work independently and manage multiple priorities.
- Comfort working with patients experiencing terminal illnesses and providing emotional support to their families.
- Ability to pass a pre-employment background check
- Ability to pass a pre-employment drug screen
Additional Informatrion
- Hours : Monday - Friday, 8 am - 4 : 30 pm
- On-Call Rotation : This is only for weeknights and typically occurs 3-4 times per year
- Hourly Rate : $35.20 - $46.00. Rate is determined by a combination of experience and education
