About The Position
Weekly Pay Range - $2032 To $2257 Job Title : Registered Nurse Job Specialty : Case Manager Job Duration : 13 weeks Shift : Day 3x12-Hour (07 : 30 - 16 : 30) Guaranteed Hours : Not specified Experience : 5 years clinical experience in an acute care setting, preferably in a med / surg environment.
License : Valid compact or state nursing license required. Certifications : BCLS certification required. Must-Have : Clinical experience, communication skills, ability to travel between locations, proficiency in using EMR systems.
Job Description : Serve as a liaison between hospital and in-home care teams to ensure patients are screened appropriately for program inclusion.
- Facilitate patient screening and enrollment into the Hospital Care at Home program in collaboration with emergency department and inpatient providers.
- Run reports in the EMR multiple times per day to identify eligible patients and review medical team documentation, documenting findings appropriately.
- Lead multidisciplinary daily huddles to discuss potential patients and available capacity.- Communicate action items to ensure timely patient transfer from hospital to home.
- Participate in rounding and unit-based huddles to identify potential candidates and educate about the program.- Manage day-to-day work responsibilities for the Hospital Care at Home program to assure optimal patient flow and satisfaction.
- Verify patient eligibility for inclusion, ensuring clinical, social, and payor requirements are met prior to enrolling patients.
- Coordinate as needed with internal stakeholders.- Introduce the program to patients, answer questions, and obtain patient consent.
- Follow up with patients and families / caregivers throughout the consent and enrollment process.- Perform assessments of patient needs, including physical, functional, psycho-social needs, and home environment.
- Demonstrate knowledge of levels of care, including observation and inpatient status, and seek input from Utilization Management as needed.
- Collaborate with all in-hospital care team members to assist with and ensure a smooth transition from hospital to home.
- Serve as a clinical point person for the in-home care team and connect them to in-hospital resources and services.- Engage other departments to coordinate services as needed.
- Educate providers, nurses, case managers, social workers, and other care team members on the Hospital Care at Home program and its benefits to patients.
- Display thorough and accurate documentation of all communication throughout the patient acquisition and enrollment process.
- Utilize interpreter services as needed.
