A company is looking for a Care Manager, Behavioral Health to provide telephonic care management services for members receiving behavioral health services.
Key ResponsibilitiesPerform telephonic care management, ensuring service needs are met through monitoring treatment plans and interagency collaborationComplete behavioral health assessments and develop Individual Care Plans in collaboration with members' providersOutreach to members for medication monitoring and facilitate care plan implementation while ensuring integration of behavioral and medical careRequired QualificationsMaster's or doctoral degree in a behavioral health field such as psychology, clinical counseling, or social workTwo or more years of related experience in Mental Health, BH Case Management, and / or substance abuse treatmentCurrent state licensure (e.
g., LICSW, LMHC, LMFT, or LADC-1 in Massachusetts)Certified Case Manager (CCM) preferredExperience with Medicaid recipients and community services is desirable
